Heavy and chronic alcohol consumption lowers testosterone in men. A large meta-analysis pooling data from over 10,000 subjects found that chronic alcohol use is associated with significantly reduced total and free testosterone. The picture is more nuanced for smaller amounts, though: low-to-moderate acute alcohol intake appears to have little effect, and some research even suggests a brief, modest increase at low doses.
In other words, this isn’t a simple “alcohol is bad for testosterone” story. The dose, the pattern of drinking, and whether it’s a single occasion or a long-term habit all change the answer meaningfully.
What the Research Shows
A 2024 meta-analysis in the journal Andrology, covering 21 studies and more than 10,000 subjects, found that chronic alcohol consumption was associated with a significant reduction in total testosterone, free testosterone, and sex hormone binding globulin, along with an increase in estradiol. Notably, this effect was seen in healthy men with regular chronic alcohol exposure, but not in men after a single acute episode of drinking, where testosterone levels were comparable to placebo (Wiley/Andrology).
A separate review in Expert Review of Endocrinology & Metabolism described a dose-dependent relationship: acute consumption of a low-to-moderate amount of alcohol was associated with a temporary increase in testosterone, linked to increased activity of liver enzymes involved in alcohol breakdown, while consumption of a large volume of alcohol was associated with a reduction in testosterone (Taylor & Francis). This helps explain why some studies on light drinking find no effect or even a slight increase, while studies on heavy drinking consistently find a decrease.
The effects of long-term heavy alcohol use can be substantial. Chronic alcohol use disorder is associated with hypogonadism, testosterone levels low enough to cause symptoms, in a large proportion of affected men. Research into the male reproductive system indicates that alcohol disrupts function at every level of the hormonal signaling pathway between the brain and the testes, not just at the testes themselves (NIH/PMC).
How Alcohol Affects Testosterone
Alcohol appears to suppress testosterone through several overlapping pathways, particularly with heavy or chronic use:
- Disrupted brain-to-testes signaling. Alcohol affects all three levels of the hypothalamic-pituitary-gonadal axis, the hormonal chain of command that starts in the brain and ends at the testes, interfering with the signals needed for normal testosterone production.
- Direct testicular damage. Chronic heavy alcohol use can directly damage the Leydig cells in the testes that are responsible for producing testosterone, reducing output at the source.
- Increased inflammation and oxidative stress. Long-term alcohol use raises inflammation and oxidative stress throughout the body, both of which are associated with impaired steroid hormone production.
- Elevated cortisol via HPA axis activation. Alcohol use increases activity of the hypothalamic-pituitary-adrenal axis, the body’s central stress response system, and the resulting rise in cortisol can further suppress testosterone.
What This Means in Practice
The clearest evidence for harm applies to heavy and chronic drinking, generally defined in research as more than 15 drinks per week, and especially to alcohol use disorder, where a large share of affected men fall into the hypogonadal range. For men who drink occasionally or in moderation, the direct hormonal risk appears to be much smaller, and some evidence suggests low doses may have little effect or even a brief opposite one.
That said, alcohol’s indirect effects are also worth considering. Even moderate drinking can disrupt sleep quality, and poor sleep independently lowers testosterone. Alcohol also adds calories that can contribute to weight gain over time, and excess body fat is itself a well-established driver of lower testosterone. So while an occasional drink is unlikely to meaningfully affect your hormones on its own, a regular heavy drinking habit works against healthy testosterone levels through multiple compounding pathways.
The Bottom Line
Alcohol’s effect on testosterone in men depends heavily on dose and pattern. Heavy and chronic drinking reliably lowers testosterone through direct damage to hormone-producing tissue, disrupted brain signaling, inflammation, and elevated cortisol. Light-to-moderate intake appears to have a much smaller effect, and may even cause a brief, minor increase. If supporting testosterone is a priority, keeping drinking within moderate limits, or cutting back if it’s currently heavy, is a reasonable and evidence-backed place to start.
